Hudson Partnership Care Management Organization (CMO) is a private not-for-profit agency that provides care coordination for Hudson County youth ages 3-21 who may be struggling with: mental health or emotional issues, challenging behaviors, substance use, juvenile justice involvement or developmental disabilities.
We use a wraparound approach to collaborate with youth and their families; to define their vision, embrace their strengths, create achievable goals and maintain success in school, at home and in their community.
The Children’s System of Care (CSOC) is a division of NJ’s Department of Children and Families (DCF). There are 15 CMOs that cover all 21 counties and NJ, under DCF. To learn more about The Children’s System of Care in NJ, please return to our Home Page and click “Children’s System of Care”
