Hudson Partnership Care Management Organization (CMO) is a private not-for-profit agency that provides care coordination for Hudson County youth ages 5-21 who may be struggling with: mental health or emotional issues, challenging behaviors, substance use or developmental disabilities. We strengthen families and their support systems to ensure that youth thrive in their community. Our goal is to see youth stay home & in school – and out of detention centers, hospitals, and residential facilities whenever possible. We utilize an evidence-based wraparound approach to individualized care coordination. The Children’s System of Care (CSOC) is a division of NJ’s Department of Children and Families (DCF). There are 15 CMOs that cover all 21 counties and NJ, and operate under DCF.
